Closer to the open date (new year), you will be invited to upload your files, complete forms, and officially continue your care at the new office.

For now, if you are genuinely interested, sign up and I will be in touch with updates. By doing this, the onboarding fee will be waived. After the office opens, the waver will no longer apply, and I will open availability to new patients.
