Comprehensive hormone evaluation and ongoing care without enrolling in our Direct Primary Care membership. Your year of care begins on your enrollment date. You do not need to wait until the beginning of a calendar year to start.

Payment options: $3,000 annually, or two payments of $1,800 at enrollment and $1,500 six months later.

Additional blood testing through Quest may be recommended to complete your evaluation. These tests are not included in the program fee and may be completed using your insurance or Quest’s cash pricing, depending on which is most advantageous for you.

No. This is a consultative hormone care program, not a primary care membership. We do not provide comprehensive family medicine services or after-hours/on-call coverage through this program. We recommend maintaining a primary care provider and will work alongside your existing providers when appropriate.

Absolutely. Dr. Burns provides hormone evaluation and management for both women and men, including menopause and andropause.

This program focuses specifically on hormone evaluation and management. If you also need ongoing primary care or more comprehensive functional medicine support, we’d be happy to discuss whether one of our Family Medicine memberships would be a better fit. Otherwise, we recommend maintaining a relationship with your primary care provider (and gynecologist, when appropriate).

Not at all. Hormone changes can occur during perimenopause, menopause, and other stages of life, and men can experience hormone-related symptoms as well. Our evaluation focuses on your symptoms, health history, and testing to determine whether hormones may be contributing to how you feel.

Treatment is individualized. Depending on your needs, we may recommend bioidentical hormone therapy, conventional medications, lifestyle interventions, nutritional support, or a combination of approaches. Our goal is to recommend the treatment that’s safest and most effective for you. We do not follow a one-size-fits-all protocol.

If you move outside Indiana, we can continue your care only as permitted by state licensing requirements. If ongoing care is no longer possible, we’ll prorate any eligible refund based on the remaining portion of your program.
